From fi urj-fin-pro *soomi, probably from earlier urj-pro *sämä and cognate with smi-pro *sāmē (see the Proto-Finnic entry for more details and references). The name originally referred only to the area now known as Southwest Finland (fi Varsinais-Suomi Finland Proper). Cognates include et Soome, izh Soomi, liv sūomõ, vot soomi and (borrowed) se Suopma.

Sananjalka, 58(1), 136–158. 37 Note however that Petri Kallio disputes that this word is an attestation of fi Suomi.Kallio, Petri. Äännehistoriaa suomen kielen erilliskehityksen alkutaipaleilta. Sananjalka 59 (2017), p. 12
The surname can be found already in a small number of sources in the 17th century, but only became common during the Fennicization of surnames in the late 19th and early 20th centuries.
